Declan Donoghue Collaborative Care Program

Summary

Collaboration among healthcare providers is critical to making diagnoses in children with undiagnosed complex medical conditions. Brenner Children’s Hospital developed an innovative program called the Declan Donoghue Collaborative Care Program in honor of Declan Donoghue who died of a rare medical condition that was undiagnosed prior to his death.

The goal of this program is to increase the collaboration among healthcare providers in an effort to make diagnoses in children with undiagnosed complex conditions.

In this session, Ms. Nicole Donoghue and Dr. Savithri Nageswaran describe why and how the Declan Donoghue Collaborative Care Program was developed, the model of collaborative care and clinical outcomes of the program.

The objectives for this webinar are:

  • To discuss the need for a collaborative model of care in making diagnoses in children with undiagnosed complex medical conditions.
  • To describe the model of collaborative care delivered by the Declan Donoghue Collaborative Care Program.
  • To describe clinical outcomes of children served by the Declan Donoghue Collaborative Care Program.

Speakers

Nicole Donoghue is Declan's Mom, patient advocate and the Chairman of the Declan Donoghue Foundation. She is committed to honoring her son Declan by creating both a medical and a spirit of play legacy through his foundation. As the author of the original request to Brenner Children's Hospital to have a new model for care in complex children's cases, she brings a unique parent's perspective to the medical community that is driven by her belief that collaboration is at the heart of every successful diagnosis. She works closely with Brenner Children's Hospital to support the promotion and success of the Declan Donoghue Collaborative Care Program.

Dr. Savithri Nageswaran is an Associate Professor in the Department of Pediatrics at Wake Forest School of Medicine in Winston-Salem, NC. She is a pediatric palliative care physician, a health-services researcher and the medical director of the Pediatric Enhanced Care Program, a pediatric palliative care and complex care program that serves children with serious and complex medical conditions. Dr. Nageswaran directs the Declan Donoghue Collaborative Care Program developed in 2011 in Brenner Children’s Hospital.
