The Importance of Near Miss Reporting

At Brigham and Women's Faulkner Hospital, 75% of the safety event reports in RL6 are near misses. Near miss events are much more common than events where harm actually reaches a patient, as much as 7-100 times more frequent. Christi Clark Barney & Tayla Hough talk about how Brigham and Women's Faulkner Hospital’s work on just culture and non-punitive response to errors has helped them have a healthy near miss reporting volume.
