02. PURSUING PROFESSIONALISM: ADDRESSING BEHAVIORS THAT UNDERMINE A CULTURE OF SAFETY AND RESPECT
When a professional’s behavior does not align with the values of their organization, this can impact organizational culture, patient safety and the learning environment. This session describes a peer based, graduated intervention approach to addressing these behaviors and essential infrastructure for supporting the effort.
03. USE OF A WEEKLY SAFETY HUDDLE AND HARM BUBBLE ON A TRANSPARENT JOURNEY TO ZERO
A harm bubble is a public display of preventable harm events in lay language, graphic format (dubbed “The Zero” due to its shape and goal of zero harm). Beverly Hospital's Zero displays 6 harm events they are actively working to eliminate, including falls with injury, c. difficile infections, surgical site infections, medication errors with harm, new pressure injuries (stage 3, 4, unstagable), and early elective deliveries. This presentation will cover the creation and results of their safety huddles and harm bubble and highlight how RL6 has supported them.
04. LEVERAGING SAFETY EVENT REPORTING TO CONFRONT WORKPLACE VIOLENCE
Memorial Health System's Workplace Violence Steering Committee identified workplace violence events were on the rise, but underreported due to barriers to reporting and lack of staff knowledge. They implemented strategies that had a dramatic impact on event reporting and has presented opportunities for change in the physical environment, work policies and staff education/training.
